Peoria, Illinois, OSF purchased Urbana Illinois Heart Of Mary Hospital and immediately ran into problems with the premier physicians in the town, Christie Clinic.  This has led to the resignation of 67 physicians from the hospital staff.  The clinic is directing patients to have care at Carle instead.  OSF wants to keep patients in their web which means air lifting them when equivalent care is available locally.  This did not sit well.  OSF also put on the discharge summary for the patient to follow-up with an OSF physician not their own physician.  Not bright hospital moves.  OSF tried to rationalize the moves but failed.  They have had to close on med surg unit so far.  

The projection as of now is for mid April for the peak of the virus to hit the hospitals.  The thought is that there will be a need for just over 64,000 total beds and 17,000 ICU beds at the peak.  

Hospitals are now ramping up their processes as to how to determine who lives and who dies.  Should they resuscitate virus patients with their dwindling supply of protective equipment.  Some hospitals are thinking of over-riding wishes of patients and not do resuscitation but would not have a blanket do-not - resuscitate order on all virus patients.  Remember the workers have to don a lot of gear prior to starting the resusitation.  Some procedures like using endoscopes or other tubes sent down a throat will send virus particle far and wide.  This is the old conundrum of doing the most for the most by sacrificing the few.  It is not so different than what I did many years ago as a member of a team that determined who got dialysis when if first came out.   

Henry Ford Health has a draft letter stating the triage for scare ventilators if necessary.  

Intermountain Healthcare is cutting the pay of their physicians, nurse practitioners and physician assistants during the pandemic.  The hospitals said this will start in June and be for those who cut their schedule due to being unwilling to be redeployed.  They will not do well in the public perception model and may lose providers after this is over.      Top

The Medical Board of California is looking into some scum physicians who are trying to make a killing on the virus.  One is selling testing kits to his rich patients for $250 each.  Another is hoping to open a drive-thru testing facility using testing kits from China and charging $500 each.  This would include a consultation and follow-up.  He is currently testing his high wealth patients "if they meet the CDC criteria.  Some believe this is unprofessional but others say it is OK to charge high prices and the health care inequities of the country allow it.  This is the same argument taken against concierge practices years ago.           Top
